Why Lean Guys Still Get a Puffy Face (And Why Cutting Harder Won't Fix It)

If you can see your abs at 7 a.m. but can't find your jaw until lunch, the problem may not be body fat at all.

Fluid settles in your face while you lie flat, so your jaw is softest at 7 a.m.

Evan is 32. He sells houses in Scottsdale, Arizona, lifts five mornings a week and scanned at 10% body fat in March.

You wouldn't know it from his yard signs.

His headshot is on 41 of them, shot at 7:30 a.m. In June a seller looked at the sign, then at Evan, and asked if he'd lost weight since the photo.

He was two pounds lighter in it.

"I can see my abs," he told me at our first session. "Why can't I see my jaw?"

His girlfriend had booked it for him.

His trainer told him to cut harder. He did, until his girlfriend said he looked sick everywhere except his face.

The guys at the gym blamed creatine. He quit for two months and lost ten pounds off his squat, nothing off his face.

His broker kept asking if he was sleeping okay. He was. He stopped booking showings before 11 a.m. and blamed the traffic.

Evan didn't have a round face at 3 p.m. He had one at 7:30 a.m.

His body fat wasn't the problem. I see the same thing in my practice every week, and I'd bet some of it is happening to you.

Check the ones that sound like you:

Tap the ones that sound like you.

Evan checked five. He skipped the beard one, then admitted he'd grown it in March.

If you checked two or more, a good part of what you're seeing may not be fat.

Body fat doesn't come and go overnight. Fluid does.

Bloat or bone? Take a side-profile selfie when you wake up and another at 3 p.m., same light. Whatever changed in eight hours isn't fat, and it isn't bone.

Your face doesn't have a drain of its own

Nobody at the gym will tell you this.

The extra fluid in your cheeks, jaw and chin leaves through your neck. About a third of the lymph nodes in your whole body, roughly 300 of them, sit in your head and neck, and that's the route out.

Drawing of a head from the side, with lymph nodes in front of the ear, under the jaw and down the side of the neck, and a line running from the cheek down to the collarbone
Your cheeks and jaw drain down the side of your neck to just above the collarbone.

Lymph has no pump. Your heart pushes your blood, but lymph mostly moves when you move, breathe and turn your head.

So the hours you spend lying flat are the hours your face fills up, even if you train twice a day.

Add a neck that's tight from screens, stress, short sleep or clenching through heavy sets, and the route out gets narrow.

That's why your face looks worst at 7 a.m. and better by lunch.

Your abs show your body fat. Your face at 7 a.m. is also showing water, and that water leaves through your neck.

Most guys never learn this because nothing hurts. A clogged drain is obvious. A slow one just looks like your face.

And that's why none of this is your fault. You've trained everything from the shoulders down, and nobody told you to look at your neck.

Why the usual fixes fade

It also explains why nothing you've tried has stuck:

None of them work on the place your face drains through.

Evan had mewed for most of a year and slept in a chin strap until his girlfriend took a picture of it.

He drew the line at skincare. "I'm not doing a 10-step routine," he said. "I use bar soap."

What lymph therapists do first

In my training, the first rule of lymphatic work was simple: clear the neck before you touch the face. If the exit is narrow, pushing on the face just moves fluid in circles.

Hospitals teach patients the same order. Self-drainage handouts start at the neck and collarbone, not the cheeks.

A printed self-drainage handout on a kitchen counter. Step 1, start at the neck and collarbone, is underlined in red.
Step 1 on a self-drainage handout: start at the neck and collarbone.

Everything that drains from your scalp, face and mouth passes through the nodes in your neck.

Studies after jaw and wisdom-tooth surgery found that drainage which started at the neck brought facial swelling down faster.1, 2

That's why I have never started a session on a client's face. Not once.

I see the same pattern in skinny guys whose bulk lands in their cheeks, in men who dropped 30 kilos on Mounjaro and kept the same jaw, and in new dads who keep hearing they look tired.

The trouble is that a session with me costs about what a nice dinner out costs, and the fluid comes back every night you lie down.

What guys like Evan need is the neck step, at home, every day, with nothing to learn.

The two-step routine

Loosen the neck. Warmth and gentle pressure on the back and sides of the neck relax the muscles the drainage route runs through.

Let it drain. Get up and move. If you like, add 30 seconds of light strokes down the sides of your neck toward your collarbone.

Do it before the 7 a.m. face walks into work, or at night before bed.

Ten minutes a day matters more than one long session a month, because the fluid comes back every night. That's why the routine has to live at home.

"I did the strokes at red lights for a week," Evan told me. "Then a guy in the next lane watched me do it, and I was done."

That's the usual story. Doing it by hand takes good technique, two free hands and a few minutes you don't have at 6:30 a.m. Most of my clients stop within a couple of weeks.

Why I started Hizoo

That's the reason I started Hizoo. It's what I sent Evan home with.

Hizoo. You lie back on it for ten minutes, and it turns itself off.

It's a heated neck pillow. You lie back on it for ten minutes, and it works the back and sides of your neck on the gentle setting. Nothing touches your face, and there's nothing to learn.

Evan's first question was the one every guy asks: "So what do I put on my face?"

My answer: nothing. It works on your neck. Your face is what changes.

What reviewers describe, week by week

It won't remove fat, change your bone structure or fix a chin you were born with. It works on the part that comes and goes.

Every night you lie down, the fluid settles again.

Every morning photo between now and the holidays catches your face at its worst hour. The Turkey Trot finish line is an 8 a.m. photo. So is Christmas morning, and the airport selfie before the early flight home. Somebody posts every one of them.

Evan started on a Monday.

On day 9 he sent me two side-profile selfies, one at 6:45 a.m. and one at 2 p.m., with no caption. For the first time, his jaw was in both.

Two weeks later his broker asked what diet he was on. He wasn't on one. He was back on creatine.

He still has a wide face. He has his dad's jaw, and no pillow changes bone. What's gone is the face that showed up at 7 a.m. and left by lunch.

Last month he reshot his headshot. He booked the photographer for 7:30 a.m. on purpose, and he shaved for it.
